Our 4-Step Peer Support Model

1. TAKING IT TO THE STREETS
Streetscape's first peer-led program, Taking it to the Streets, is a peer-led, low-barrier, housing-focused, community-centered street outreach that leverages our community trust to build the trust and relationships critical to getting individuals experiencing homelessness engaged in health and wellness services and housing while addressing food insecurity and providing access to essential resources necessary for daily living, including a pet pantry and free animal pet care clinic twice a year.
For those ready to embark on a new chapter, we provide on-site housing assessments and applications to available housing lotteries and referrals to health and wellness resources, coordinated entry, and local sheltering programs.
Taking it to the Streets also serves as a vital space for STREETSCAPE peers to have a reliable place to routinely meet and engage with previously homeless individuals who have secured stable housing and to locate individuals for time-sensitive appointments, including housing.

2. TWO-WAY STREET
Two-Way Street is the second step in Streetscape's peer support programs. Individuals experiencing or previously homeless voluntarily enroll in this program by consistently engaging with our peer support services. Two-Way Street fosters the growth of peer-to-peer relationships through shared experiences and a deeper understanding of each person's story. We utilize tools like motivational interviewing to support individuals in achieving self-identified goals, such as maintaining appointments with service providers or succeeding in housing. Program enrollees gain access to person-centered resources, including new jeans or housewares.

3. STREETSMART
STREETSCAPE's third peer support program, Streetsmart, is designed for individuals progressing in their housing journey while still experiencing homelessness.
Streetsmart program participants voluntarily engage in a more intensive peer support model, which guides those in need in navigating the complexities of subsidized housing. Peers assist in obtaining necessary documents and signed releases of information, aid in apartment searches, and offer trusted peer advocacy. Through shared experiences, Streetsmart provides a supportive and understanding ear throughout the sometimes lengthy and confusing housing process.

4. BEAT THE STREETS
STREETSCAPE's fourth peer support program, Beat the Streets, provides person-centered wrap-around peer support for previously homeless individuals who have secured stable housing. Beat the Street program enrollees voluntarily engage in one-on-one peer support and small peer groups to address daily living adjustments and connect with local resources.
Beat the Streets provides a space for sharing experiences, building resilience, and exploring new paths to health and wellness. Enrollees receive a Beat the Streets t-shirt and a Welcome Home Kit with essential household items. Many continue engaging with our street outreach program; some become volunteers and future Streetscape peers.

STREETSCAPE Peer Support Services and Outreach has an active partnership with our Continuum of Care (CoC), Metro Denver Homeless Initiative (MDHI), and have obtained the training to enroll our clients in the Homeless Management Information System (HMIS), Coordinated Entry (CE) and provide immediate person-centered housing assessments (SPDAT). We also assist in local housing lottery applications when available.
STREETSCAPE collaborates with local nonprofits, faith-based groups, and agencies that align and enhance our mission and vision.
STREETSCAPE peers possess lived experience with homelessness who have successfully maintained their housing, without incident, for at least one year and have been trained in SAMHSA best practices of person-centered, trauma-informed, and strength-based approaches in Peer Recovery Coaching.
